Guest Safety & Waiver

A parent, guardian, or responsible adult is responsible for supervising the child and ensuring that Celebrate rules and staff instructions are followed.
Grip socks are required for play areas where posted or directed.
Active play, inflatables, toys, bikes, and play equipment involve inherent risks such as falls, collisions, bumps, strains, and other injuries. I understand and voluntarily accept those inherent risks for my child.
I will tell Celebrate staff before participation about any limitation or circumstance that requires special attention for safe participation.
To the extent permitted by Oregon law, I release Celebrate from claims arising from ordinary negligence connected with participation in recreational activities or use of the facility. This does not apply to gross negligence, reckless, willful, or wanton conduct.

I am the child’s parent/legal guardian or I am authorized to sign for this child, and I have read and agree to this waiver.
