OPEN PLAY AGREEMENT

Open Play Waiver

Please review once. One parent/guardian signature completes the waiver for this registration.

Supervision. I am responsible for supervising my child(ren) at all times during Open Play.

Play & Safety. I understand that active play, inflatables, ride-on toys and games involve ordinary risks of bumps, falls and injury. My child(ren) will follow Celebrate staff instructions and posted safety rules.

Grip Socks. Grip socks are required for participating children.

Health. I will not bring a child who is sick, contagious, or unable to participate safely.

Personal Property. Celebrate is not responsible for lost, stolen or damaged personal items.

Emergency Care. If I cannot be reached in an emergency, I authorize reasonable emergency assistance for my child(ren).
